Why timing dominates the result
Testosterone has a circadian rhythm, injected esters have a release curve across the interval, and both effects are large enough to change a clinical conclusion. A draw taken the morning after an injection and a draw taken the day before the next one are measuring the same protocol at two different points on a curve — and they can differ enough to look like two different protocols.
You cannot remove that variation. You can standardise it, which is the entire point of thinking about timing.
Before therapy: morning, fasting, repeated
For diagnosis, the Endocrine Society guideline describes measuring fasting morning total testosterone and confirming an abnormal result by repeating the measurement. That is not bureaucratic caution — biological variation and assay variation are both large enough that acting on one low value risks acting on noise.

On therapy: trough, mid-interval, or simply consistent
Two conventions are in common use for men on injectable esters. Many clinics standardise on a trough draw — immediately before the next injection is due — because it is unambiguous and easy to repeat. The Endocrine Society guideline discusses measuring serum testosterone midway between injections for men on enanthate or cypionate. Both are defensible; your prescriber's protocol is the one that counts.
What is not defensible is drifting between them. If you drew at trough last time, draw at trough this time. Record the date and time of your last injection on every panel; without it, nobody can tell where on the curve the number came from. How often: the monitoring windows guidelines describe
Cadence is set by your clinician. For orientation, guideline documents describe a pattern along these lines:
- 3–6 months after starting — testosterone and hematocrit reassessed, with symptoms reviewed.
- Annually thereafter — the same core markers, assuming things are stable.
- First year — particular attention to prostate monitoring in men for whom PSA follow-up is appropriate; the AUA guidance also puts haemoglobin and hematocrit measurement and polycythaemia counselling before therapy begins.
- After any protocol change — long esters need weeks to reach a new steady state, so an early draw measures a system still in transit.

What quietly ruins a draw
Most uninterpretable panels are uninterpretable for mundane reasons. Worth noting on the day, and worth mentioning to whoever ordered the test:
- A different laboratory or a different assay — a new series, not a continuation. This applies especially to free testosterone and to estradiol.
- A different time of day, or a non-fasting draw when the previous one was fasting.
- A moved, missed or doubled dose in the weeks before — the curve you are sampling is no longer the one you think.
- A recent blood donation or phlebotomy — directly relevant to hematocrit and haemoglobin. See the hematocrit guide.
- Acute illness, a hard training block, or significant dehydration before the draw.
- High-dose biotin supplements — the FDA has documented interference with a range of immunoassays, testosterone and estradiol included.
None of these are reasons to skip a panel. They are reasons to write down what was true on the day, so an odd result can be explained rather than chased.
What to bring to the appointment
A clinician reading your panel is trying to reconstruct context you already have. Arriving with it written down is the single highest-leverage thing a patient can do:
- Date and time of your last dose, and the dose itself as prescribed.
- Your actual adherence over the period — missed and shifted injections included.
- Previous panels, ideally from the same laboratory, with their reference intervals.
- Any donation or phlebotomy, and when.
- Current medications and supplements.
- Symptoms with dates, rather than a general impression.
