TRT blood test timing: when to draw your panel

Why timing dominates the result

Testosterone has a circadian rhythm, injected esters have a release curve across the interval, and both effects are large enough to change a clinical conclusion. A draw taken the morning after an injection and a draw taken the day before the next one are measuring the same protocol at two different points on a curve — and they can differ enough to look like two different protocols.

You cannot remove that variation. You can standardise it, which is the entire point of thinking about timing.

Before therapy: morning, fasting, repeated

For diagnosis, the Endocrine Society guideline describes measuring fasting morning total testosterone and confirming an abnormal result by repeating the measurement. That is not bureaucratic caution — biological variation and assay variation are both large enough that acting on one low value risks acting on noise.

On therapy: trough, mid-interval, or simply consistent

Two conventions are in common use for men on injectable esters. Many clinics standardise on a trough draw — immediately before the next injection is due — because it is unambiguous and easy to repeat. The Endocrine Society guideline discusses measuring serum testosterone midway between injections for men on enanthate or cypionate. Both are defensible; your prescriber's protocol is the one that counts.

What is not defensible is drifting between them. If you drew at trough last time, draw at trough this time. Record the date and time of your last injection on every panel; without it, nobody can tell where on the curve the number came from. What quietly ruins a draw

Most uninterpretable panels are uninterpretable for mundane reasons. Worth noting on the day, and worth mentioning to whoever ordered the test:

None of these are reasons to skip a panel. They are reasons to write down what was true on the day, so an odd result can be explained rather than chased.

What to bring to the appointment

Common questions

Should I get my TRT bloodwork at trough?

Follow the protocol your prescriber uses. Trough draws are common because they are easy to define and repeat; guideline text also describes mid-interval sampling for men on cypionate or enanthate. Either way, be consistent and record when your last injection was.

Do I need to fast for a testosterone test?

Guideline text for diagnosis describes fasting morning samples. On therapy, follow the instructions from whoever ordered the panel — and keep the conditions the same between draws so your results stay comparable.

How long after changing dose should I retest?

Your clinician sets this. As background, long esters take roughly four to five elimination half-lives to approach a new steady state — weeks rather than days — which is why guideline reassessment windows are described in months.
