High hematocrit on TRT: monitoring, phlebotomy and blood donation

Hematocrit is the marker most likely to interrupt a testosterone protocol. This guide covers what it measures, why therapy pushes it up, the thresholds written into clinical guidelines, and where voluntary blood donation genuinely differs from a medical procedure.

What hematocrit measures

Hematocrit is the proportion of your blood volume occupied by red cells, reported as a percentage. Haemoglobin — the oxygen-carrying protein inside those cells — is measured on the same complete blood count and generally moves with it. Both are routine, cheap and reported on nearly every panel.

They matter on testosterone therapy because testosterone stimulates red cell production. When that stimulation pushes red cell mass above the normal range the result is called erythrocytosis (older reports say secondary polycythaemia). The FDA-approved Depo-Testosterone label instructs that "haemoglobin and hematocrit levels (to detect polycythaemia) should be checked periodically in patients receiving long-term androgen administration."

Why testosterone raises it

The review literature describes several contributing mechanisms rather than one: a shift in the erythropoietin set-point, suppression of hepcidin with greater iron availability for erythropoiesis, and direct stimulation of the bone marrow. Ohlander and colleagues' 2018 review of erythrocytosis following testosterone therapy notes that the effect is formulation-dependent, with "short-acting injectable formulations having the highest associated incidence" — consistent with higher peak exposure driving the response. The same review is careful to note that evidence linking therapy-induced erythrocytosis to vascular events remains limited.

The thresholds clinicians work from

These figures come from guidelines written for prescribers. They are included here so you recognise them when they appear in your notes — not as numbers to act on yourself.

Note what none of these say: they do not tell a patient to stop therapy, donate blood or change a dose on their own reading of a number. Every one of those actions is a clinical decision, and an elevated hematocrit can also prompt a search for other causes such as sleep apnoea or smoking.

Blood donation is not therapeutic phlebotomy

These two get conflated constantly, and they are different things.

Some men on testosterone therapy donate blood, and some are advised to have therapeutic phlebotomy. Which — if either — applies to you is a question for your clinician, not a conclusion to draw from a lab report. Repeated red cell removal also depletes iron, which is why ferritin often ends up on the panel alongside hematocrit.

Common questions

Is a hematocrit of 52% high?

It depends on your laboratory's reference interval, your baseline, your altitude and your clinical picture — and interpretation is your clinician's job. What you can do is make the number comparable: same lab, same timing, and a note of anything that shifts plasma volume, such as dehydration before the draw.

Should I donate blood to lower my hematocrit?

That is a clinical question, and this page will not answer it for you. Donation is governed by donor-eligibility rules and exists to supply other patients; managing an elevated hematocrit is a medical matter your prescriber should direct — potentially via therapeutic phlebotomy rather than donation.

How often can you give whole blood?

Under 21 CFR 630.15, a whole blood collection may not occur more than once in 8 weeks — 56 days. Double red cell apheresis carries a longer deferral, commonly 16 weeks at US centres. Individual collection centres may apply stricter rules.
